Information to include in the referral
You can use this page to refer yourself, a family member, someone you support professionally, or a client who may benefit from care at home.
Once we receive your referral, we will review the information, contact the relevant person and discuss whether Shine Care Agency can meet the identified needs.
- Name, age and address of the person needing support.
- Main care needs, including personal care, medication prompts, meals, mobility, companionship or domestic support.
- Any diagnosis or condition that affects care, such as dementia or a physical disability.
- Current risks, including falls, mobility, continence, nutrition, medication, safeguarding concerns or hospital discharge needs.
- Preferred visit times, frequency and duration if known.
- Whether support is needed as visiting care, live-in care, night care or another arrangement.
- Funding details, such as private funding, local authority funding or mixed funding.
- Contact details for family members, representatives or professionals involved.
- Consent to share the information and to be contacted about the referral.
Urgent and emergency referrals
We will do our best to respond promptly to urgent referrals, including hospital discharge or short-notice care needs. In some urgent situations, an initial assessment may be needed before a full care plan is completed.
